Care planning is a process that helps to ensure that people receive the care and support they need to live as independently as possible. It is a way of thinking about and organising care and support so that it meets people’s needs and preferences.
The four key steps to care planning are:
1. Assessment – This is when someone assesses what care and support you need. This could be done by a social worker, doctor, nurse or other health or care professional.
2. Care plan – This is a written document that sets out what care and support you will receive and how it will be delivered.
3. Implementation – This is when the care and support in the care plan is put into place.
4. Review – This is when your care and support is reviewed to make sure it is still meeting your needs and preferences.
